Do I qualify?

You qualify for CareOregon Advantage Plus HMO-POS D-SNP if:

• You have Medicare Part A and B.
• You have full Oregon Health Plan (OHP) Medicaid benefits.
• You live within the service area: Clackamas, Columbia, Jackson, Multnomah, Tillamook and Washington counties in Oregon. Click here to view a service area map.

You will receive all your Medicare benefits — including Part A, Part B and Part D (prescription drugs) — from CareOregon Advantage when you enroll in our Plus plan.
